For me, it's usually realizing I didn't put the insulator on the wire before crimping the connector.

I'd just say that the first flare was practice.

I hate when I do that. I almost did that the other day on a gas line at a customers house. It would have been too short to reflare!

Long ago a friend did this with a multicable (24 channel audio mixer cable)  i saw he was halfway soldering those little wires into a connector without first putting the cover on the cable, as the other side was already done.. he bad mouthed for an hour (!) and took all those wires off to start again!  24 earth covered in stereo so 72 wires to solder in 1 connector... lots of FUN.. 🤬

At first i told him to stop.. but i had to persuade him to do so.. he didn't see it.. but after i showed him the cover that was lying around he started cursing!    It was SO FUNNY !!

Gas lines, always good to make a curl in them.. say it's for taking up vibrations!  😉
